A wise old concrete mason once told me "I only know two things about concrete. It's grey and it cracks". I agree with most everything said here (5" not 4" and rebar not mesh) but would add that you must control the cracking that will certainly happen. Concrete expands and contracts especially on cold climates. Control joints will make it crack only where you intend. This can be done two ways - During pour (only by qualified, experienced mason) or cut in after with a diamond blade conc. saw (your local rental yard). If you do the saw cut, it should be done ASAP. That evening if possible as night time temp changes can make the cracking ocur overnight. Joints should be laid out in a square grid 8x8 is ideal but 10x10 should be okay and work well with your 40x60 size. They should be 1/4 the depth of the slab (1-1/4" for a 5" slab). If you are concerned about water geting into these cracks, they can be caulked to seal. As said by others, preparation is the key. Cause there's one more thing I know about concrete, it gets real hard!
