Real life experience - Brother has 98 Cummins 12 valve 2500, engine has been bulletproof after the dowel pin update. 5-speed transmission has been rebuilt three times now at 170k, first two were 5th gear nut falling off, last one was a bearing going out and running through ALL the gears ($1750). Pickup is holding up just fine and still makes 18 to 22 mpg regularly unloaded. Ran it a couple days hauling hay about 5 miles round trip and letting it run all day and managed 15mpg out of it.
His 06 Cummins runs like a scalded dog - way more power than you need and the 6-speed has close splits to get you moving with a load. Mileage is lower (16 mpg) than the older one.
