In all the discussion of this event I have never heard any mention of a soft plug. I was of the opinion that all wood /coal boilers had a soft plug in the center of the crown sheet that would melt if the water got low and the temperature got to high. The resulting release of steam would smother the fire and cool things off before a major event could happen. I have operated loco type boilers and and have never had this happen but I imagine that there would be considerable soot, ashes and sparks from the stack if the soft plug melted. I think that there would be plenty of time to take appropriate action if the operator had been watching his sight glass and wasn't trying to stretch things to the limit. I vote for operator error in this case. No pun intended. Farmered
