8N belt pulley / oil pressure

When taking belt off for alternator I noticed there are two locations to put the belt on the drive pulley. Why. Is this for aux. implement / hyd? Oil pressure 40 cold and 25 hot. HOw bad is the engine? Thanks for the help.
