Current code is to run separate ground and neutral from the main disconnect to an outbuilding. The ground conductor needs to be grounded at the outbuilding as well. The old code allowed only three conductors at the oubuilding, but still required a separate ground rod at the outbuilding and required ground and neutral to be kept separate at the outbuilding.

As for mobile homes, I'm pretty sure the code has required four conductors for more than 27 years.
