Nothing. The starter won't care about the polarity. (A starter generates two magnetic fields. If you flip the polarity of both fields, the direction of rotation doesn't change.)

As far as I know -- the alternator most commonly used is a Delco 10SI. This alternator requires negative ground.
There are also 6v alternators available (in both positive and negative ground versions).

The kits are undoubtedly really easy to use.
If you decide to roll your own, John CO sells brackets for this conversion. Or if you're handy with a welder, they're easy to make. Assuming a Delco 10SI is used:
The stud on the back of the alternator case connects right to the battery.
Looking at the two terminal connector from the rear of the alternator with the connector at the top of the case -- the right-hand wire goes to the alternator stud (the one you connect to the battery).
The other wire from the two-terminal connector should be connected to a resistor, diode, or lamp and then the other side of the resistor, diode or lamp is connected to the ignition switch. Need a 12v battery connected in negative ground.
Any lights need to be replaced with 12v lights.
Any other 6v equipment (a radio?) needs to be replaced with 12v versions. And you're done!
