May be as simple as the pulley brake needs adjusted tighter so as the pulley does not go back too far when disengaged as to pinch the back facing again. I'd also take it apart, grease the bearing and check for broken facings which can be VERY dangerous. If broken they can stack in there wedging the clutch tight which means you won't be able to stop when most needed to.
