Or if it's worn out. Over years of use mine has worn to the point where it's has a rather large oval hole in it. The two bolts that hold it in place are long enough for you to put a 3/16 inch or even 1/4 inch doubler plate over the front to act as a bearing surface provided you have a way to get a snug fitting wing-nut shaped hole through it. (I used the "file and fit" method.) This is for the A, as Steve mentioned.
