My Great Uncle was had a harrow hooked up to his fenderless Case, he went to turn and didn't relize he had caught the cable with the lug on the tire, just missed takeing his head off by very few inches. Had a neighbor who collected tractors when I was a pup, he went to start a 4010 at the starter, had it in gear and the wheels were turned toward the side he was on, ran over him and keep going in circles and just kept running over him until it ran out of fuel. Its just like when you're backing up to hitch onto an Impelment and you have someone back there (usually my dad) waiting to put the pin in, I always keep a hand ready for the ignition switch if something ever went wrong.
