I got my form from the link listed below and it is still there. I just checked. The address to send the form to might be hard to read so I will include the mailing address here. The cap is black and measures 2 1/2" high and 2 15/16" in diameter. It resembles a miniature oil filter for a car. My thanks to the person who made the form available to us....
