I just bought a 1987 GMC 7000 dump truck with a 5 speed trans. it makes alot of noise in all gears except 5th. also when you take it out of 5th and try to go back it grinds and will not go like it is not syncronized , it will go to 4th real easy and then back to 5th. with no problem.

If the transmission is direct drive in 5TH (as opposed to some that are overdrive in 5TH) noise in gears other than the direct gear often indicates a bad bearing.

When you are in 5TH, if it is direct, the power simply flows straight through the transmission topshaft, from front to back, and the countershaft and gears are not carrying a load, so if there is a bad bearing, it doesn't make noise because it's not loaded.

How's the oil level? It could be low, or the synchro's could be going bad, if it's not shifting properly. It's also possible that there are some clutch issues developing, and the clutch is not fully disengaging. That is often less noticeable in higher gears.
I can't say that the last one I drove was very nice to begin with. I'd much sooner drive any roadranger than a syncronized medium duty transmission. Mabey this one is just ignorant to shift? Don't know.

Sounds like your clutch is not fully disengaging. I don't know for sure if those have an adjustable clutch or not. It's worth having a truck shop look at.
