Three washers are correct. ANy decent machine shop can rework the head. I have Jim at our local Harley shop do my head work. Have them check it for cracks too.

The studs that take the 3 flat washers are the ones that are closest to the centerline under the rocker arms. There are steel sleeves pressed through the water jacket of the head casting and these washers spread the clamping force of the nuts over a larger area to hold sleeve and cast iron around it stationary.
From a manufacturing standpoint, drilling the head and inserting sleeves would produce better overall results than having troublesome core and casting defects.
