350 ut fan in the radiator again don't know why

I have a 350 ut and for some reason the fan went in to the radiator( cut the bottom tank right along the top next to the veins) .I can not find nothing wrong with the fan all I could see is it looked liked the fan blades where bent out. This happen one other time when the bearings in the fan hub went bad .but that is not the case this time. Does any one have a ideal what may be cuseing this . I would like to stop this from happing again .This is a costly fix. radiator are not cheap to buy or fix.
At lest it happen now and not when I was in the field with it.

It could be that your radiator mounting bolts are loose and letting the radiator bounce up and back
