NH Baler Slip Ratcheting Clutch

Have a NH 311 I was baling with today. Heard a clunk and baler stopped working. Got back to the house with it (dad finished with 2nd baler). Found one pin from ratcheting slip clutch laying in guard and center bolt and washer still sitting in pto shaft. Appears to thread back together.

Going to check functionality tomorrow. My question is WHY? Why would this happen? Never happened before...

Just weird. Has been probably 12 years since I got the baler..,I kind of remember this happening early on.

Has been one of my most trouble free years...
I was just getting used to not holding my breath...
 
Function of time and repetition. Baler is man made, nothing man made is perfect or last forever. Thus as long as man made machines are used they will require repair and repair men to complete these repairs. Also preventative maintenance is an added measure to the equation. Now knowing of this possible failure checking the item described as a pin in this case for being properly secured before use can go a long ways towards continued reliability of the machine.
