That looks very different from the Ford conversions for which I have seen the old ads. It is apparently a high crop so they probably sold few.
 
Very neat.
Being a Model T guy, I noticed a couple of things.
That is a Model T front axle that has been bent to that shape.
Made from vanadium steel which was very strong even when bent.
Pre 1916 radiator, but a later steel firewall.
The early ones were wood.
Must be a later engine with the coils mounted on the side instead of on the firewall.
Sure would like to see more photos from the side and rear.

That rear drive seems familiar. I have seen a picture of that drive setup before. I think somebody offered a kit configured like that to
make the model T into a 2 wheeler that could pull horse drawn implements with the operator sitting on the implement seat.
 
It might be.
If it is not an original axle, it definitely been added to using another Model T axle.
Definite Model T frame and engine and transmission and rear end.
Even has the Model T hubcaps on the front spindles.
