Here's what happens when ya forget to put the dump down

How does that happen?

I could see it being left up after unloading, but how would it get raised with grain still in it?
 
Could the trailer have lifted gradually as he drove- hard to make a turn without seeing that/flipping it over.
 
happened to my uncle with a single axle dump--the pto lever got hit by something on the seat and it engaged while going down the road
 

Every dirt dummie's nightmare. Last summer a new driver at a friend's company pulled out from a side road onto the main road. Not only did he run the stop sign, but he had the body still up and enough momentum to take six utility poles down. He went very quickly from new driver to former driver
 
Just leave the pto engaged and drive. That's what I did. Sooner or later it will go all the way up.

I don't get it, are you saying there might have been a reason for the pto to be engaged before he even started this trip? I can't speak for all dump trailers, but shouldn't there be more than one action to raise the bed? Simply forgetting to disengage PTO or accidentally engaging should not in itself raise the bed, even then maybe there should be a bright warning lamp that illuminates when engaged.
 
I expect the lever got engaged somehow going down the highway,anyone with a license wouldn't leave a yard with the dump still up,it sure would be quite a shock having that happen,
 
Got to see the aftermath of that at a wood pellet plant, dump
truck left hauling pellets. Bed started to rise, took down a lot
of phone lines before he hit the big site power line that didn?t
break and flipped his truck over.

Yes foreplay, you need to have both the lever for the hydraulic valve in the raise position for it to go up, and either in the raise or hold position for it to stay up, then you have to have the PTO lever in on, and every PTO lever that I have ever seen in modern, maybe newer than 15 year old trucks, requires that the driver hold a button in the center of the knob down with his thumb while he moves the lever. Thus the PTO lever could not have been bumped to move, and the valve lever would have to have been moved as well. My take on it is that he purposely raised it to that height, then got interrupted, perhaps a text from his GF, then forgot the body and took off.
 
Our 2016 dump trucks are doing it on their own(roads dept). We have pumps that are running all the time(one less component to fail) and with these great electronics and components the hydraulic companies are coming out to replace the faulty spool valves, allowing the box to go up by itself. The driver can be blamed for not watching the mirrors, but who loads the trailer with the box up, like some are assuming.
 
I have no idea but I would have thunk that there is a warning light or buzzer on when ever the box is off a limit switch and tranny is in gear.

That tarp must be very strong to keep the load in the trailer at that angle. Or maybe the load was frozen?
 
About 40 years ago I came down I-70 just after that happened on a side road in O'fallon, Mo. A long dump trailer hauling gravel to a ready mix plant pulled out of the dump ramp with his bed still up, and stuck the trailer in the 3 phase lines that fed about 1/3 of O'fallon's business district. The lines stopped him. When the truck started to burn he opened the door to get out, but at least had sense enough to realize to get clear of the truck before touching the ground, and they said he cleared that truck by nearl 15 feet before he hit the ground because of how hard he jumped.

Bright warning lights only work for awhile, then they go out and are never fixed. If the PTO is left on, the hyd valve can jiggle and vibrate itself into the engaged position if it’s manually run by a lever. That’s why you see some of them held back with a tarp strap. Watching the mirror can be easily forgotten about for a few miles.
