WE have a very large fireplace that takes up one whole wall in our home--there is a wood insert in there now. We do not use it.
QUESTION-----There is a small square door on the outside, but looking from the inside I can't see that door. Want to run a gas line thru that door into the fireplace area, but can't see how it can be done. Figuring you fireplace experts would know the answer......I understand that small square door is probably for when the chimney is cleaned etc.....
 
I'm gonna go with Jim on this one. The old cast doors were either ash doors or coal doors and by your description I doubt it's a coal door. put on some gloves, grab a small shovel and explore. My guess is you'll find what you are looking for.
 
It's a vent.

Should be a grate at the back of the fire box, down in the bottom.

Theory is, you can close off the front doors tight, then it draws air from the outside instead of drawing heated air from the inside.

You should be able to fish a flex line up, drill a hole down through the fire brick if necessary.
 

It is probably a clean-out. On fireplace chimneys clean-outs have to be above the smoke shelf which will be about 1.5 -2 feet above the top of your opening into the room. The smoke shelf is there to reduce down drafts. Clean-outs are usually around 8x10 inches.
