Keeping real wheel tight

I'm having the darndest time keeping the real right wheel tight on my Massey 255. It has the 4 bar adjustable wheels on it and we've used a 3/4" impact as well as a 3/4" breaker with a pretty good sized cheater pipe and it still works loose. Anyone got any suggestions, I'm out of things to do to it. Keith
 
The bolts are all new (just replaced them last fall. I'm thinking new wedges too. Thanks guys for the replies. Keith
 
The wheels are like the AC Power Shift, with 4 rails? Rails are likely worn if slippage has occurred.
Can dismount tires and build up with weld, or, build up the wedges with weld, but then they fit in only that wheel width position. I"d do the rails. BTDT.
 
I had that problem on a Case tractor,rear wheel bolts would work loose no matter how I tightened them..one day I had enough and I bought new bolts and nuts..never came loose again,been tight for almost 17 years now
 
I have a tractor I seldom use, and when I get it out the rear wheels have a flat spot from sitting. For a while they have a bad surge, back-to-forth,or a forward-back-forward-back push-pull motion, driving along, til the flat spot works out. Might you have something like that, working back and forth??
