Oil in coolant/water in oil

Are the causes of oil in the coolant the same as water in the oil in say something like a Perkins V8? Is this likely to be cylinder head gasket issue or more likely to be a seal in the oil cooler?
 
In my experience if you have oil in the coolant it is a gasket issue with the oil cooler or a junk cooler and if you had coolant in the oil it would generaly be a blown head gasket or possibly a bad o-ring on a cylinder liner if it has wet liners.
 
Oil in the coolant would indicate a leak somewhere where the oil pressure is greater than water pressure. A rotted oil cooler will do it. A blown head gasket isn't likely to introduce oil into the cooling system.
