When I contacted MK Martin a year ago, they said that they had stopped manufacturing the bale baskets due to lack of demand. I ended up getting the EZ-trail baskets.

I did a google search for them. Found all kinds of distributors for them, but couldn't actually find the manufacturer. Here's a number listed for one of the distributors: 1-800-287-7999 They may be able to give you more details.

MK Martin builds Forage King silage boxes and hay elevators . . . same company that makes the bale baskets?

I'd never heard of these boxes before. The ones I know (from my post below) were built by a small company in Ridgeland, WI that has been defunct since the mid '80's. Also built running gears, kicker baler racks and manure spreaders. Don't think they made bale baskets, though.
