H popping out of gear

My father-in-law has a '46 H that wants to pop out of 5th gear. I remember reading about the problem on here, but can't seem to find those posts. Is it a bearing problem or a gear problem? Thank you for any replies.
 
Most likely it is a spline wear issue. The 4th 5th sliding gear is a "gear in 4th only in 5th it couples the input shaft to the output shaft for direct drive. (as in many standard transmissions.)
the beating the splines take from grinding from 4th to 5th is the cause. But with a load on a wagon, that is SOP. Try bates equipment for replacement gear/and input shaft. The intermediate pilot bearing in between the shafts can also be worn/broken allowing it to walk out of gear. The ball and spring detents can also be rusted and non functional. Last item is a bent/worn 4th 5th shifter fork. Some of these can be found together contributing to the issue. Jim
 
9 times out of 10 the gear is rounded off from shifting from 4th to 5th. Remove top cover then you can get a real view.

Like the other said they don't have a lot of power in 5th gear, pulling a load in 5th gear from a stop is a problem so folks figured out how to get it rolling in 4th and bang/force/grind/jerk/powershift it into 5th to get home. After 60 or 70 years of this abuse you see problems like rounded over gear teeth, bent shifting forks and the like. Add in some normal wear and tear and you might have to do some tranny work.
 
About nine out of ten times for the ones I have worked on replacing the main shaft front bearing cured the problem of jumping out of fifth. As long as the bearings do not have excessive play the gears can be quite worn before they cause problems.
