BD154, 3414 overheating

I rebuilt my bd154 diesel last year with a replacement new head. For some reason it overheated and blew out the concave freeze plugs in the new head. I rebuilt it this year again and it’s doing the same thing. There are two freeze plugs in the top of the head under the rocker shaft. One or the other of these have blown out a few times now. The bad thing about it is all the coolant ends up in the oil and it costs about $40 to flush out.
I have a rebuilt radiator, new water pump. Any help.
 
There are bolt in place welch plugs that cure that issue. They expand and lock in place very positively. Google welch plugs to find sources. Jim
 
Not bragging but I have rebuilt a lot of these engines over the last 30 years and not had this problem. Try checking the thermostat for sticking or the radiator for being blocked. The ensines I worked on were in Manitou fork lift trucks wich weighed 5 tons with a lift capicty of 2.5 tones and went back and forward all day on farms and building sites. Was your new cylinder head made in India or China, the Indian ones are good but know nothing about the Chinese ones. MJ
 

I found some brass bolt in plugs at napa made by Dorman. Also my thermostat was new last year. I guess its possible it may be upside-down?
There is also a tank heater installed on it. There is a "tee" in the lower radiator hose that goes to the plug in canister kind and around to a nipple on the starter side of the block. Do you think this could be causing bad cooler flow and can I cap it off?

I'm not sure were the head was made. But I had the head milled and rebuild this summer. And they replaced all the frost plugs with new ones. The radiator is record. And for the termostat maybe I'll leave it out this time and see what happens.
