Possible causes are pressure plate levers not adjusted properly, failed throw out bearing, or worn out coupler bolts and plate at the rear of the clutch.

I have to assume the part you are talking about is the throw-out bearing carrier, I also have to assume that one of the ears on the carrier is what is broken, or cracked. It is also unlikely that the two replacement parts are defective, but I doubt that is the case. That being the case I would guess that there is a problem with the linkage from the clutch pedal to the throw bearing. Is the bearing lubricated properly and does the bearing run smoothly on the transmission tube? Are the pressure plate fingers adjusted properly. Is the pressure plate installed correctly? Is there about 1/8" clearance between the bearing face and the pressure plate fingers with the clutch released? Is it possible the clutch is going too far forward and the ears are contacting something? I think you can see that without seeing a picture(s) or being there it is somewhat difficult to say what the problem is.

Thanks for the help it turns out that the shaft going back to the tranny was not seated in the pilot and that is what was wrong.
