Super M lost a fan blade

I have been trying to bring back a Super M to life. Actually got it started today. Even though the throttle was set to low, the engine revved up. By the time I hit the kill switch, one blade of the fan broke off, It took out the shroud and radiator. What would cause a fan to break a blade off?
 
Most likely the blade had a fatigue crack from thousands of hours of constant flexing. Then the added centrifugal load from overspeeding stressed the remaining metal at the crack until it gave way and the blade broke free.

I'll bet if you closely check the other 3 blades you'll find each has evidence of a hairline crack near the hub.
 
I watched an "expert" get a barn find M running once. He brought along a carb, battery, and small remote gas tank and fixed up the ignition. Put a squirt of oil in each cylinder and She fired right up and proceeded to run at a speed no M was ever designed to see. The fan did exactly what yours did and then broke the old belt and as he ran around trying to pull off his hotwire rig #2 rod and piston made a new inspection hole in the side of the block.
 
If you had the carb off of the tractor, the small tang and slot system on the tube connecting the governor to the carb throttle is not in its correct place. If you didn't, the throttle was left in the open position when it was shut off, and the throttle shaft stuck in the carb, leading to dramatic overspeed. The throttle shaft movement can be checked with removal of the front cup with tube cover on the top of the governor, or by removing the screws and pulling part way open the connection at the carb. Sorry about the destruction, I hope that was all that got wounded. Jim
 
Thank you for the words of wisdom. I removed the fan belt this morning and saw that 2 blades were missing. Im glad I found the second before my mower did. The cause was that the rebuilt carburetor was miss adjusted. I backed out the idle adjustment screw and every thing calmed down when I cranked it. Fortunately no other damage occurred. What is the procedure for removing the radiator? I understand the 2 bolts. How do you remove the steering shaft?
 
The front sheet metal is removed, then the large nut on the front of the steering gear box is removed (lefty loosy) then the steering shaft screws out of the box (turning it causes it to moove past the sector gear and it is removed out the front. The only worst case is if someone welded the universal yoke to the shaft. Then it may be necessary to grind off the weld to get the shaft through the radiator. Jim
