340 Utility (gas) problems

I still can't figure out what caused the hydrolic gear to grind off half the teeth on the cam gear all the way around. Has anyone had a problem like this with the 340 or possibly 240 utilities? I assume that the hydrolic gear locked up some how and because that gear is hardened, it ground down the cam gear. I took the hydrolic pump off and the gear spins freely. Any ideas?
 
A couple possibilities:

1 - Overloaded hydraulic pump. Look for the wrong relief valve installed (pressure set too high) or the relief valve missing and replaced with a plug. (A dead-headed hyd pump will lock up solid without a relief valve.)

2 - Wrong cam gear installed. I'm not familiar with the 340 - but a live pump installed on an H or M without also swapping in a heavier cam gear can destroy the stock gear. Could your situation be similar?
